Individual Community Living Support Services

We provide the programming and service that people with home health care need to live life on their own terms. We offer personalized services in both community and in-home settings to support individuals and their families. Available through waivers and other funding sources, our services are designed and delivered by professionals with compassion and respect.

A person is eligible to receive individual community living support services with approved EW plan. It includes the following service categories:

• Active Cognitive Support
• Adaptive Support Service
• Activities of daily living (ADLs)
• Household Management.
• Health, Safety and Wellness.
• Community Engagement.
